After Action Forms

 Following operations that included actions against submarines or aircraft, Pacific Fleet standardized report forms had to be completed and forwarded up the chain of command.  The Sumner's files in the National Archives do not contain any examples of these forms.  We thank Anne Mansell Moodey who forwarded us the following samples from the records of the USS COOPER (DD-695).
